Overview

The 2024 Enhancing Statewide Falls Prevention Awareness, Strategy, and Collaboration grant is intended for a single organization. Its aim is to develop and strengthen collaborations between state falls prevention coalitions. Through this, the goal is to reduce falls or the risk of them among older adults, adults with disabilities, and their caregivers.

The eligible organization must have specific capabilities. Firstly, it should be able to quickly execute and manage up to 12 competitive sub-awards. Secondly, it should offer technical assistance to a group of state falls prevention coalitions. This support should be focused on initiatives that increase the quality and effectiveness of falls prevention activities, leading to proven outcomes for older adults and adults with disabilities.

Another requirement for the eligible organization is to ensure each coalition is inclusive. They should bring together a varied group of stakeholders to jointly identify state and community needs. These teams will then generate and distribute necessary resources and education on falls prevention. In addition, ensuring long-term infrastructure and sustainability for these efforts is also part of the organization's responsibilities.

Finally, the organization must design an evaluation to measure the effectiveness of these coalitions in reducing falls or the risk of them. This will help understand the impact of the coalition's efforts and make necessary adjustments or improvements.
