The "Implementing Patient-centered Delivery of Clinical Preventive Services in Primary Care (PCOR TF)" is a project that aims to enhance the fair distribution of high priority clinical preventive services to adult patients. This initiative will rely on research-evidence, putting the patient's needs and expectations at the heart of primary healthcare. This patient-centered approach will utilize multiple strategies such as cooperative team-based care, maximizing technology and informatics, and establishing connections with public health and other community organizations. This project will also focus on integrating patient preferences and values into the models of care and ensuring care coordination. One of the major objectives of this project is to minimize healthcare disparities in order to promote equality in access and service delivery.
