Overview

The Health System Collaboratory for Chronic Disease Management (U54) is a proposed initiative aimed at advancing national care standards for chronic diseases, including for those managing multiple chronic conditions. Their primary goal is to enhance access, improve care quality, and address health inequity by disseminating and implementing Patient-Centered Outcomes Research (PCOR) evidence.

This grant, if awarded, would fund affiliate health systems, facilitating their participation in a learning collaborative alongside other health systems. As part of this collaborative, each system would take on the implementation of one or more interventions suggested by the Agency for Healthcare Research and Quality (AHRQ). These interventions, all evidence-based and high-priority, aim to enhance care provision for chronic conditions within primary care practices affiliated with these health systems.

In addition to implementing these interventions, selected health systems would commit to teamwork in order to devise implementation strategies. This collaboration would also foster nationwide learning and sharing of best practices in implementation. Moreover, these health systems would undertake testing and evaluation of various implementation strategies to ensure their effectiveness.
