Overview

The Physical Activity and Nutrition (PAN) Unit aims to decrease racial disparities in health outcomes by supporting communities in implementing community-led initiatives in the focus areas of food security, built environments, healthy children and youth and social cohesion. Across the state, there are communities disproportionally affected by chronic disease, poor health outcomes, food insecurity and physical inactivity. Research suggests that social and economic factors, the physical environment, and health behaviors have a larger impact on health outcomes than clinical care. For many years, the PAN unit has been working with communities on policy, systems, and environmental change strategies to improve health. While this work has been beneficial in increasing food access and supporting active communities, disparities still remain. To reduce these disparities and address the main drivers of health outcomes, MDHHS needs to truly invest in communities. Funding needs to allow the community to identify and implement solutions that will work for their unique needs. The Healthy Community Zones is a place-based initiative that provides funding to Chippewa and Saginaw Counties and the City of Detroit. Data suggest these communities have higher rates of chronic disease prevalence and mortality. Investing in these communities provides the opportunity to change their health trajectory and build an environment that promotes good health and a place where community members can thrive. Funded projects will adopt evidence based and practice tested strategies known to improve the area(s) of focus and that can last beyond this grant. MDHHS will provide technical assistance to support local communities as they work to create and implement comprehensive long-term strategies for a healthy, thriving community. Additionally, MDHHS will support grantees in evaluating the success of their strategies to encourage further investment.
