Overview
The focus of the initiative is to strengthen statewide HMP and SNAP navigation, outreach and eligibility support capacity by leveraging health and human service providers, nonprofit organizations, other community-based navigation partners and a flexible navigator workforce. The successful applicant will establish a coordinated statewide navigation network by conducting a competitive bid process to identify secondary partner organizations to serve as community navigators.
Eligible applicants must be 501(c)(3) nonprofit organizations.
The initial implementation period is anticipated to run through Saturday, Sept. 30, 2028, and may be extended through Monday, Sept. 30, 2030. MDHHS expects to award one agency approximately $7.25 million. Project director requests to access the application are due by 5 p.m., Thursday, Sept. 24. Grant applications must be submitted electronically through the EGrAMS system by 3 p.m., Wednesday, Sept. 30.
A pre-application conference will be held to provide instruction on using the EGrAMS system. The pre-application conference will be held at 10:30 a.m., Wednesday, Sept. 16, and will last approximately 90 minutes. The webinar can be accessed at https://bit.ly/3Uy4Bi3. At the conclusion of the conference, a recording of the webinar
will be posted on the EGrAMS system.
