The purpose of the program is to provide consultation services to assess patients for substance use disorder (SUD), deliver motivational enhancement therapy, psychoeducation, recovery planning and relapse prevention, provide care coordination and linkages to SUD services, provide peer recovery coaching and engage community-based providers to better meet the needs of patients with SUD.
