Health Fund’s Healthy Aging Grant Program

The Health Fund's Healthy Aging grant program invests in programs, partnerships, and models that equip organizations, health care providers, and policy makers to effectively meet the needs Michigan residents face as they age. The program aims to better integrate Michigan's strong network of aging programs into the broader health care landscape, increase the capacity of partners, and generate new solutions to fill critical gaps where they exist.

Eligible applicants include nonprofit organizations and public agencies serving Michigan. Applicants must have an active account in the Health Fund's Fluxx grants portal; new users are required to request an account in advance of the deadline to guarantee access.

Eligible projects focus on improving access to care, helping residents age in place, supporting caregivers and families, addressing behavioral health, improving nutrition and food security, and building more age-friendly communities that function better for older adults.

The application process requires submission of a concept paper by March 30, 2026, followed by a full application due June 4, 2026. Applications must be submitted through the online Fluxx grants portal. Awards are announced in November 2026.
