Community Health Impact Program

The Community Health Impact (CHI) Program was designed to support health-focused, community-based organizations and local agencies tackling health disparities, advancing health equity, and building community wellness in close partnership with Michigan communities. As of 2026, the Michigan Health Endowment Fund no longer awards grants through the standalone Community Health Impact Initiative, having integrated its community-driven approaches as a cross-cutting framework across other initiatives such as Healthy Aging and Healthy Kids.

Eligible entities historically included tax-exempt nonprofit organizations, federally recognized Tribes, local units of government, and the State of Michigan. Organizations were required to maintain at least one paid FTE and provide an IRS tax-exempt determination along with audited or independently reviewed financial statements or Form 990. While applicants were not required to be headquartered in Michigan, all funded project activities had to take place within the state.

Eligible projects focused on addressing social drivers of health, elevating community voice, strengthening local healthcare access and navigations, and advancing equitable community wellness. Applications were previously submitted through the Michigan Health Endowment Fund online Grant Portal following published requests for proposals. The program is now closed to new applications.
