Community Health Impact Program

The Community Health Impact Program was designed to support health-focused, community-based organizations tackling health disparities, advancing health equity, and building community wellness in close partnership with their communities. The program invested in community-driven, place-based initiatives addressing the drivers of health, medical respite capacity, eviction prevention, health navigation, and collaborative community wellness councils throughout Michigan.

Eligible applicants included nonprofit organizations, public agencies, local health departments, and educational institutions in Michigan. Preference was given to initiatives demonstrating strong community voice, collaborative local leadership, and sustainable community empowerment models.

Eligible projects involved evidence-based or promising innovative interventions that addressed social determinants of health and improved health outcomes for underserved populations.

As of 2026, the Michigan Health Endowment Fund is no longer awarding grants through the dedicated Community Health Impact Initiative. Instead, the community-driven approaches developed through the program have been integrated as cross-cutting principles across the Health Fund's other grantmaking areas, including Healthy Aging, Healthy Kids, Behavioral Health, and Capacity Building.
