Overview

Goal/purpose: The Community Health Impact program supports health-focused, community-based organizations with bold ideas that empower communities to solve their most pressing health challenges and implement community-driven interventions that build stronger, healthier communities across Michigan. Additional applicant eligibility: Applicants must be IRS tax-exempt, serve Michigan residents, have a current audit or independent financial review, and at least one paid FTE; the program prioritizes grassroots, neighborhood, and small to medium nonprofits, and larger organizations are less likely to be funded; the primary applicant should be a community-based organization. Project eligibility: Projects should target a defined place or population and focus on eliminating health disparities, improving health equity, building community wellness, increasing social integration, strengthening community support systems, and expanding health-related community engagement; example topics include oral health access, health literacy, health-related transportation, and environmental health; proposals aligned with the Health Fund’s other initiatives (Nutrition and Healthy Lifestyles, Behavioral Health, Healthy Aging, Maternal and Infant Health) are not considered here; ineligible costs/activities include clinical research, loans, health-related emergencies, capital projects and major equipment (including vehicles), litigation, ongoing program operations and staffing, lobbying, tuition/fees, and work by organizations that discriminate. Application process: A required two-page concept paper was due by email by 5:00 p.m. on 06/04/2025; by invitation only, full proposals were due through the grants portal by 5:00 p.m. on 07/15/2025; awards are announced in November 2025 with the earliest start date of 12/01/2025.
