Overview

The Section 32p(4) Home Visitation Grants are allocated to Intermediate School Districts (ISDs) or consortia of ISDs to implement home visitation services. The purpose is to support families and children birth through age 8 experiencing risk factors, with goals to improve school readiness, positive parenting practices, and family economic self-sufficiency. Applicants must be ISDs or consortia of ISDs serving as fiduciaries for their Great Start Collaborative (GSC). Projects must implement evidence-based home visiting models (such as Parents as Teachers, Healthy Families America, Nurse Family Partnership, Play and Learn Strategies, or Early Head Start-Home Based) as part of a locally coordinated, data-driven strategic plan. Applications must be submitted via NexSys by September 15, 2024, including required work plans and model fidelity documentation.
